Context
Patients forget 80% of what doctors say.

Not because they don't care.
Not because they aren't capable.
But because the moment they walk through that clinic door, the system stops working for them.

For the "Young-Old" (Active Seniors aged 50–70), managing health has become a second job. Despite being tech-literate, this demographic is often underserved by digital health platforms that prioritize clinical data over human comprehension.

Bridging the handover gap
Supporting senior autonomy by transforming complex medical consultations into actionable adherence plans.

The Memory Gap: Clinical anxiety measurably suppresses short-term memory retention during the 15-minute consultation window — the exact moment where compliance is decided.

Fragmented Ecosystems: Post-visit instructions scatter across paper summaries, MyChart portals, and pharmacy apps — creating a fragmented, low-compliance handover with no single source of truth.

To gain a holistic view of the healthcare 'handover,' we synthesized quantitative data from 74 seniors with qualitative insights from 5 active seniors and 1 primary care physician, bridging the gap between patient experience and clinical reality.

To bridge the gap between raw research and actionable strategy, I used Affinity Mapping to synthesize data from 74 survey participants and 5 deep-dive interviews. By clustering hundreds of qualitative observations, I identified four strategic pillars—Cognitive, Emotional, Systemic, and AI Opportunity—transforming fragmented user pain points into a structured roadmap for design intervention.

Cognitive: "Information Accessibility ≠ Semantic Clarity."
Data is visible, but the intent is invisible. Real accessibility requires translation, not just larger fonts.

Emotional: "Dignity is the Engine of Medical Adherence."
Active seniors disengage when tech feels like a "crutch." Autonomy must be designed as a core feature, not a byproduct.

Systemic: "The Care Gap Lives in the Handover, Not the Clinic."
Medical failure happens in the 'Post-Visit Fog'—the transition from clinical expert to independent home manager.

AI Opt.: "AI as a Cognitive Safety Net."
Generative AI's value isn't 'more data'; it's offloading the stress of memory so the user can focus on human connection.

Cognitive Empathy over Data Density: Prioritize "Human Speed" processing. Instead of displaying exhaustive clinical data, focus on semantic clarity to prevent cognitive latency.

Dignity-Driven Autonomy: The AI should act as a "Cognitive Safety Net" rather than a restrictive monitor. Design for empowerment, ensuring the user feels in control of their health roadmap.

Minimalist Hierarchy: Reduce "Information Noise" by surfacing only what is immediately necessary, while keeping detailed records accessible but secondary.

Mapping the Cognitive Handover
Mapping the Cognitive Handover
Information accessibility does not equate to semantic clarity. In the high-stress environment of a medical consultation, seniors often experience 'Expert-to-Human' latency—where complex data becomes a cognitive burden rather than a guide.
To solve this, I designed an Information Architecture that prioritizes minimalist hierarchy. The system is built on two primary pillars: Memos and Chats. By keeping the Ambient Sync and AI Chatbox as persistent global actions, I ensured that the tool to capture or clarify information is never more than one tap away.
This architecture transforms high-entropy medical dialogue into low-stress, prioritized household instructions.

Information accessibility is not the same as semantic clarity. This Wireframe is designed to strip away clinical noise.
By translating complex data through a structured interaction loop, we bridge the communication gap—converting overwhelming medical summaries into a prioritized hierarchy of "Next Steps" that anyone can follow with dignity and zero guesswork.

Ethical Onboarding: A streamlined prompt ensures verbal permission is obtained before recording.
Live Synthesis: Real-time transcription captures the dialogue, allowing the user to focus on the conversation rather than taking notes.
Instant Decoding: The system automatically extracts and explains medical jargon in the moment.
The Result: Seniors can instantly grasp complex terms, enabling them to clarify details with their provider on the spot and ensuring no instruction is lost in transition.

As the Product Designer & Strategist, I spearheaded the 0-to-1 development of this project, owning everything from initial user research to systemic architecture. By operating at the intersection of Design and Product Management, I ensured that every creative decision was anchored in business viability and technical feasibility.
Recognizing the fragmentation of the North American market (Payer/Provider silos), I steered the product from an "all-in-one" platform to a specialized “Post-Consultation Cognitive Tool.” This strategic shift simplified technical execution while solving the most acute pain point for elderly users.
Key Learnings
AI value in healthcare isn't about content volume; it's about reducing uncertainty. The Role of Design: Design must act as Cognitive Compensation, bridging the gap between complex medical data and patient understanding to foster absolute trust.

Synthesized insights from orthopedic specialists, GPs, and nurses to translate clinical rigor into design hierarchy. To mitigate patient confusion over post-op contraindications, I implemented high-priority visual labeling and mandated that expert medical orders override AI suggestions to ensure clinical safety.
